SCUBA-CCR LEARNING AND ATTENDANCE AGREEMENT

Welcome to your PADI™ Diver Course or Event. Diver training is meant to be fun and exciting, but like any form of learning, you will need to invest some time and effort to make the most of your course. In an effort to offer you and your classmates the best program possible, please review the following and indicate your acceptance at the bottom. This learning agreement, between you and the Northwest Diving Institute, reviews our mutual course responsibilities.

Academic Responsibilities

Our SCUBA-CCR programs are home study in nature. Review sessions with an instructor are designed to answer questions, and review key points, NOT AS A SUBSTITUTE FOR INDEPENDENT STUDY. To facilitate learning, please:

• Review course orientation and open water training hand-outs received upon enrollment.
• Review course orientation and open water training hand-outs received upon enrollment.
• Review and complete student record forms and medical questionnaires.
• Have medical form signed by a physician if necessary.
• Be on time for class.
• Be prepared for class by reading/viewing the required chapters in your book, DVD or CD-ROM.
• Complete the necessary knowledge review(s) for each session, as required by your instructor, which will be collected during each training session.
• Ask questions about anything that is not understood or need additional help with.

Confined and Open Water Responsibilities

Confined and open water training is the part that we all look forward to; it is here that you learn the skills necessary to be comfortable and confident in the water. To make most efficient use of your time please:

• Be on time for class.
• Follow all procedures as outlined by your instructor (where to arrive, where to leave equipment, etc.).
• Have mask, fins, snorkel, boots, gloves, and any other personal equipment prepared for all training sessions.
• Follow all course procedures as outlined by your instructor, and be attentive during your training sessions.

Late Arrivals - Not Being Prepared - Cancellations - No Shows

If the student arrives to class late, or without their completed assigned work, it may be necessary to make up the work and continue the class at a later date. The student will then be responsible for any additional costs and/or inconveniences this causes. In scheduling and determining additional costs, we agree to give every reasonable consideration to unforeseen events, such as family emergencies, that lead to this situation. However:

• Makeup fee for missed class/pool session: $35 per recreational; $50 per technical; and $75 per CCR session.
• Makeup fee for missed open water dive: $50 per recreational; $75 per technical; and $100 per CCR dive.

Please also note that group classes adhere to a set course schedule. Should you and your instructor find that you are unable to keep pace with the group or a make-up session as described above, transferring to private lessons may become necessary. No Shows for single classes result in loss of fees paid, and for non-fee events result in a six month suspension of sign-up privileges.

Instructor/Staff Responsibilities

If the student completes all course work assigned, arrives for class promptly, and otherwise follows directions for learning given by the instructor, we accept responsibility for learning challenges. Course objectives must be met before the student is certified. Your instructor and staff agree to:

• Be ready to start the class as scheduled.
• Provide a positive and fun learning environment in which to master course objectives.
• Answer the student's questions to the best of their ability.
• Be patient and assist the student through learning challenges.

I also understand and agree that all outstanding balances will be paid in full prior to commencing studies.
